Model for Improvement Worksheet

The Model for Improvement is a simple, yet effective model that assists organizations and improvement teams clearly focus on the endpoint of what they aim to accomplish and improve. The model informs the QI approach from both a “thinking” and “doing” perspective.

The `thinking` component encompasses the three fundamental questions that guide the improvement journey—1) What are we trying to accomplish?, 2) How will we know that a change is an improvement?, and 3) What changes can we make that will result in an improvement? The `doing` component speaks to the continuous process of developing and conducting small tests of change (Plan-Do-Study-Act or PDSA cycles).
